Bayside High School

In order to try out for any team, students MUST obtain a Department of Ed Medical Form from the office of the Athletic Director, Mr. Capuana, in room 100, as well as a permission slip; fill out both and return. During the first week of classes, coaches post signs throughout the hallways indicating the dates and times of tryouts.

Below are forms that need to be completed in order to participate in FALL, WINTER, and SPRING PSAL Sports. If you are interested in participating in a sport, please contact the coach.
